Ameritas 2025 Per App Bonus Program

$15 for each eligible individual dental case. (Spirit Dental, Magnum Dental and Ameritas Primestar® dental plans qualify)

Cases must be enrolled between 9/1/25 and 12/31/25 and be effective between 9/2/25 and 1/1/26.

A minimum of 15 eligible dental cases must be issued during the contest period to qualify for the bonus.

Dental cases are eligible for bonus payments after the second premium payment has been applied and the policy has remained active for two months.
